New Patients:  Please fill out the registration form below to join our Collective.  In the message please tell us how you prefer to be contacted.  After we verify your information with your doctor we will contact you to set up a time for delivery. Registration by phone is also available Monday-Saturday from 10am-9pm.

All new patients must be verified before any orders are processed.  No exceptions!
